When the Smile Squad team schedules a visit to your school, you'll need to fill out a consent form before students can receive dental care. The consent forms on this page are just samples. Your child's school will provide the official form. This form provides parents and carers with information about Smile Squad services and helps Smile Squad gather the details it needs to provide personalised care.

If the patient, or their parent or guardian, agrees to the services and cost, they must sign this consent form.
