The What Matters Most Discussion Starter helps older people think about what would be right for them if they became very sick or were nearing the end of life, before a crisis happens.

These resources support health and aged care workers to have important conversations with older people, and to include their family and friends in the discussion.

The Discussion Starter includes questions about the person, their health, and what they may want at the end of life. Health or aged care workers can use these questions to guide a supportive conversation and help the older person explore what matters most to them.
